Ah, but Sam leaps his whole body in - the topology of his form is modified by the residual aura of the person he's leapt into, but the actual physical form remains his.
The baby's probably in suspended animation until the curse is removed.
I had a cool character background where a person was turned to stone while pregnant and the child was eventually born safely by breaking the statute open. In D&D it's less about what is biologically accurate and more about what would make a cool story :)
In this case, the C in C-section stands for "chisel."
It could go the Zeus-Dionysus route: the baby gets stuck somewhere bizarre for a little while. But if it's in suspended animation, this would be a great way to postpone the birth of an heir/prophesied chosen one/etc.
Well, let us first begin by considering what happens to all of the gender specific features of the character during this time.....IE, when a male character puts on the girdle of femininity...where do his genitals go?
Wherever they go, the baby goes the same place, I would imagine.
I don't think it's fair to assume they're transported somewhere else and "kept in storage", as it were. My impression is that one set of genitals...uh...morphs into the other set.
Well, that's sort of what I mean....I would just say that whatever you think happens to genital flesh, it's likely that the same thing happens to fetus flesh, ie the fetus tissue morphs along with the uterus into....um....whatever it's morphing into.
Simplest seems to be fetus=genitals for the purpose of this. But really, I have no idea what I'm talking about and just talking because, well, it's kinda fun.
The fetus and uterus "morph" into a beer belly. Then there is no matter transfer physics to get into. Okay, there would still be some matter transfer physics to deal with, BUT there would be no matter loss issues.
"Okay. You buckle on the Girdle of Masculitity, securing it beneath the taut skin of your distended belly. There's a feeling of vertigo as your flesh slides over itself, reconfiguring your features to those of a man. In a moment it's over. You feel the way your weight is distributed more around your shoulders and less around your hips, and your blouse now hangs flat across your absent breasts.
But your nutsack hurts like hell and you have an Austrian accent."
No, honestly, I think it would fail. The power of the deity of birth/life/whatever is used in this campaign would be stronger than the magic of the belt.

What should happen, then?

Nothing happens to the mother...
